By telephoning us on 01325 461128 between the hours of 8.00am and 6.00pm Monday - Friday. Alternatively by visiting the surgery in person or by using SystmOnline (by firstly obtaining a username and password from us).
By telephoning us as early as possible in the morning when the surgery opens at 8.00am. Home visits are strictly for patients who have mobility issues attending the surgery. If you cannot attend the surgery for an appointment please contact us to discuss.
By telephoning the surgery on 01325 461128 after 11.00am. The receptionist will ask you a few questions to ensure that she is dealing with the correct result. Please note - receptionists are not medically trained and may therefore not be able to go in to great detail as to what each result means. If you would like to speak to a Doctor regarding your test results, please ask the receptionist who will arrange this for you.
To change your address a form must be completed which can be obtained from visiting the surgery in person or by printing out the 'Change of Address' form and returning it to us in person or by post. A Change of Name request requires proof of the change which has been made e.g. marriage certificate. Please visit the surgery bringing along any certificates you have, in which the receptionist will then take a photocopy and ask you to complete a 'Change of Name' form.
Please visit the surgery in person to collect registration forms and questionnaires which involve your past medical history in which will be passed to the Doctor. We encourage patients to register with their nearest practice and so your address will be asked upon registration. Please also bring proof of address and identity upon registration with the surgery.
